Evan enjoys caring for patients with chronic kidney disease at our Forest Lake and Edina clinics, as well as at dialysis units throughout the metro area. He’s passionate about enhancing understanding of kidney disease and its treatments, and strives to tailor care so that it best fits into patients’ lives. He received his bachelor’s degree from Gustavus Adolphus College, and his master’s degree from Rosalind Franklin University of Medicine & Science in 2011.
Erin Gottesleben, PA-C
Erin sees patients at our Edina clinic, as well as several dialysis units in the south metro. She is interested in all aspects of acute and chronic kidney disease, dialysis and medication management. Erin gets to know each person as an individual and works to design treatments with each person's primary goals in mind, utilizing shared decision-making. Erin obtained an undergraduate degree from University of Wisconsin–Madison and master’s degree in physician assistant studies at Methodist University.

Jacquie’s love for healthcare and helping others started at a young age. At Northwestern College in Iowa, she received her undergraduate degree in athletic training. After working in kinesiology for several years, she returned to school and graduated from Bethel University’s Physician Assistant program. Now, as a physician assistant at InterMed Consults, Jacquie enjoys the spectrum of kidney disease and management. She greatly appreciates building long term relationships with patients and accompanying them in their kidney care journey. Jacquie sees patients at our Edina clinic, as well as dialysis units throughout the metro area.

Melissa Hall, PA-C
Melissa is a board-certified Physician Assistant specializing in Nephrology. She earned her Bachelor's degree from the University of MN, Twin Cities, and her Master of Health Science in Physician Assistant Studies from Quinnipiac University. Her clinical practice focuses on optimizing outcomes in our advanced-stage chronic kidney disease patients, as well as her Sub-Investigator role in our clinical research studies.

Sara Taylor, PA-C
Sara earned her bachelor's degree from the University of Wisconsin-Madison and her master's degree in physician assistant studies from the University of Iowa. She began her career as a Physician Assistant in Infectious Disease in 2007, went on to spend several years working in Interventional Radiology and Vascular Medicine, and ultimately returned to her primary focus and passion: Infectious Disease. Sara provides patient care for the inpatients at Fairview Southdale Hospital as well as for the outpatients at the Edina clinic.
